Other Info I have already found out that my birth mother, Ann Alice Boyette aka Ann Cook is deceased. I am seeking other relatives of hers to find out info on my birthfather. There is no name listed anywhere for him. My sister and I have already reunited. We had the same birthmother but different birthfathers. My birthfather may be Hispanic, African American or Italian-not really sure. I have a photo of my birthmother, but no way to scan it into the computer. I need to find someone who knew her between 1953 and 1960 to get information about my sister's and my birthfathers or any information on other relatives.

Other Information My birthmother’s name is Ann Alice Boyette (aka) Ann Cook. She was born on 8/12/29 at St. Ann’s Hospital in Cleveland Ohio at 8:44am. Her parents names were John H. Boyette and Cecelia Coll (maiden name). They were living at 1253 E. 86th Street in Cleveland when Ann was born. (not sure of zip code) John was born in Samson County, NC and was a metal worker at L. S. Cutler Co. Cecelia was born in Indianapolis, IN and was a housewife. John and Cecelia were married in Cuyahoga County in Cleveland Ohio on 12/29/26 by Rev. William E. Degan. John was living at 1364 E. 112th St. and his parents names were John K. Boyette and Condia Sutton (maiden name). Cecelia was living at 1177 E. 123rd St. and her parents names were William J. Coll and Mary A. Campbell (maiden name). Ann spent time in Rosemary Home as a child and was sent to Columbus State School in 1944. Ann had filed for a marriage license on 1-31-55 to marry Charles Henry Bainer, but the license was returned on April 11, 1955 as "Never used". Ann used an alias when I was born because she had a hard time when she gave my sister up for adoption. She didn’t want anyone to know that she was surrendering another child. My sister was born on 12/28/54 at Booth Memorial Hospital in Cleveland OH and her name given at birth was Patricia Ann Boyette. Her name now is Patricia Roloff (I have already located her and we are very close!) I was born on 4/27/57 at Mt. Sinai Hospital in Cleveland OH and the name given me at birth was Betty Cook and my birthmother signed the birth certificate Ann Cook. My adoption papers gave the only real piece of evidence to my true identity. It lists Name of Child before adoption: Infant Girl Boyette, aka Betty Cook. Until my sister and I were reunited I always thought that Boyette was my birthfather's name. Ann was 27 when I was born and living at 6300 Linwood Avenue in Cleveland OH. Ann worked for a widower with three children as a homemaker. My sister and I had information that she had married the man she was working for but have recently discovered she never married. Ann passed away on June 19, 1987 at Ohio State University Hospital in Franklin County, Columbus, Ohio. Her address at that time was 3691 Eakin Road, Apt #11. She died of respiratory arrest, pulmonary edema, pneumonia, end stage valvular heart disease and renal failure. The Long Funeral Home located at 5528 Cleveland Avenue in Columbus, Ohio handled her funeral arrangements and she was laid to rest in Holy Cross Cemetary which is in Indianapolis, IN. My sister and I are sad that we never had a chance to know our birthmother, never could hold her and comfort her. She had such a hard life. Other Information The person I am looking for may be a Hispanic, an African-American or maybe even an Italian man who knew a woman named Ann Alice Boyette sometime between 1955-1959. Ann would have conceived me approximately Aug or Sept of 1956. She was supposedly in Cleveland, Ohio maybe visiting while away from the State School in Columbus. Ann described my birthfather as "being about 5'6" tall, thin, having wavy black hair and dark skin". Ann began prenatal care in Oct of 1956. She suffered from a severe heart condition called: mitral stenosis insufficiency. My birthfather may not even know that I exsist.
